Central Florida Health Alliance offers you two ways to receive special perks and amenities that will enhance your experience when you are a patient at Leesburg Regional Medical Center and Alliance First Care at Tavares. The cost for the Living Well program is a $10 annual fee for membership, with a portion of the proceeds benefitting the Alliance Healthcare Foundation. Refunds are not available.

Healthy Express

Offered FREE, your personalized Healthy Express card identifies you quickly in our family of healthcare services as one of our unique patient participants. Your complimentary Health Express card provides you with:

• Expedited registration upon admission with the swipe of your card – an instant and safe patient identifier
• Monthly health e-Newsletters with health education and wellness information
• Medication record card for your wallet to track what medications you are taking

Living Well

Our premier, upgraded membership program that will enable you to receive the benefits mentioned in the Healthy Express program information above PLUS:

• Priority invitations for health screenings and events (2 per year)
  
• Local merchant discounts with your membership card
• 10% discount in the cafeterias and gift shops at Leesburg Regional Medical Center

The cost is a $10 annual fee for membership to Living Well with a portion going to the Alliance Healthcare Foundation, which supports the services of Central Florida Health Alliance.
